If you would prefer to fax or mail in your questionnaire, print out the pages from this format instead of the Online Questionnaire. These pages should print out exactly the same as the hard copy questionnaire I am handing out in my "target district".

These pages are PDF files which require you to have the free Adobe Acrobat Reader installed on your computer. Note: There are 17 pages, which includes an introduction to the questionnaire and the SFU Ethics forms. Part B takes up the most paper because I have left big spaces for you to write in!!!

For "my input" here, I highly recommend that you download the Acrobat Reader. Most Government documents (eg. the I.R.P.s!) and many other resources are now being offered in PDF files. The Acrobat Reader program is free. The part that isn't free is the program Adobe Acrobat 4.0, which allows to you to make PDF files. (That's what I had to purchase in order to produce a printable copy of the questionnaire. I spent ages "fiddling" and still could not get the pages to line up correctly online. I bought the program and within a very short time had everything beautifully ready to go! This research study is definitely getting me to do some "new learning" about technology!)
